Scheduling your first appointment is simple and easy. You can schedule an appointment yourself by visiting our contacts page. You can schedule by calling us at 408-905-9897 or emailing us at info@chrysalischiropractic.org.
Remember, we are open on weekends! Our office hours are Thursday-Monday from 9:00 AM to 7:00 PM. Emergency appointments are available for Tuesday and Wednesday upon request.
Your first visit will be a 1 hour appointment that will include a thorough history of your health, physical exam, and treatment. Please dress comfortably and bring any health information that may be important for us to understand your health, such as X-rays, MRIs, radiology reports, and lab tests.
We only answer to YOU, the patient, because that is the only way we can provide YOU the best care you deserve. Oftentimes, insurance companies like to control the type of care you receive, by approving what forms of treatment they want to pay for. We want you to decide what will be best for you as we present the most effective options to benefit you on your health journey to better yourself. At Chrysalis Holistic Chiropractic, we want you to be in control of your own health.
We are an “out-of-network” clinic, so we can happily provide you with a “superbill” that includes all necessary information (diagnosis codes, treatment codes, fees, etc) that your insurance company may need in order to reimburse you. Please call the customer service number on your health insurance card to learn about your out-of-network chiropractic care benefits.
“Superbills” will also be acceptable by your Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) as “health-related expenses.”
Medicare: We accept Medicare as a “non-participating physician,” meaning we collect the full price for your visit charges at the time of service, then send your Medicare billing in for you. After reviewing your coverage, Medicare and any of your secondary insurance policy will reimburse you by sending you a check in the mail.
Payment for each visit will be required at the time of service. This will include any examination and treatments fees, as well as any payments for supplements or products that may be necessary for your care.
We accept cash, check, and all credit cards.
*There is no out-of-network reimbursement for Medicare, Tricare or Kaiser/HMO patients.
